The shrunken world of the twins





Some real life situations are so much infelicitous that makes our soul ache for long. Few days back, there was this twin (Aakriti and Prakriti; name changed), just 6 years old, who came to OPD with their father.

They both had similar refractive errors BE in the range of +12.00Ds. They were legally blind under the WHO definition (BCVA of better eye was ≈2/60). They have nystagmus, and they have RP.
It is more painful to let the father know that their condition is progressive. We read theories in books, but theories are not going to change their lives.
Many of us who prescribe high hyperope might not have worn any glasses. We only have read about the physical and optical characteristics of such lenses. We prescribe as if that is our work. But have we attempted to feel a life behind those thick lenses?
Certainly the blur circle of real world is brought to a point focus or simply minimized in size but the window (lens) through which their eyes peep to the outside is rendering things larger and appear nearer. If the worldly objects are bigger, their world would be shrunken than ours.
Look at this picture in which her clear world is constricted so much around her body. She remains inside a virtual plus twelve diopter enclosed cell.

High-dose vitamin A supplementation (e.g. palmitate 15000 units per day) probably has a marginal benefit, but caution may be advisable in light of potential adverse effects, notably the increased risk of lung cancer flagged by the Age-Related Eye Disease Study (AREDS) in smokers taking betacarotene, hepatotoxicity in susceptible subjects and worsening retinal function in some genetic subtypes of RP; it should be avoided in pregnancy or planned pregnancy. Several other drugs (e.g. calcium-channel blockers) have shown potential benefits but their efficacy and safety in RP have not been fully ascertained.
